ADVSD Benefits and Enrollment RFP

ADVSD is pleased to announce a Request for Proposals for a new program within Community Services focused on serving seniors (older people), and people living with disabilities who are eligible for a variety of benefits including the Supplemental Nutrition Assistance Program (SNAP), assistance paying for Medicare, Medicaid and energy assistance.

The goal of the RFP is to help connect historically underserved populations in Multnomah County to existing services and benefits they may be entitled to through a culturally responsive outreach and enrollment process.  The funding opportunity also includes age-friendly programs, which require experienced and trained volunteers to provide case management support for our Public Guardian clients improving their quality of life and staffing County-designated cooling centers on short notice.

Populations of focus:

  • Younger adults with disabilities,
  • Communities of color, specifically Latino/a and Asian (Vietnamese, Cantonese, Mandarin, and Korean speaking) communities for whom English is a second language, 
  • African American, Native American, and Hawaiian/ Pacific Islander adults living in poverty, 
  • Medicare eligible LGBTQ and Gender non-conforming adults of all ages,

Using Multnomah County Aging Disability Resource Connection (ADRC) database to record interactions and make referrals, the staff and volunteers will connect to existing services and programs either within ADVSD or with other community partners.

Estimated Timeline: RFP released June 30, 2017 and closes July 31, 2017. An optional pre-proposal meeting is Monday, July 10 from 3:00 - 5:00pm at 421 SW Oak St., Portland, Lincoln Building, Oak Conference Room (first floor).

Estimated Funding: $174,955 annually. In-kind contribution required.

Contract: initial 2 years with option to extend for an additional 3 years
