Criminal background check

You can submit a completed Background Check Request Form to our downtown office at :

You must submit the form in person if it is your first time or if you your background check has been expired.

For both offices:

Cash, debit, credit cards are NOT accepted. Please bring a Check or money orders (made to the Adult Care Home Program) for all fees. 

Bring a current, government-issued photo ID. 

Background checks which you have received for other purposes are NOT accepted. You will need a new background check even if you live or work in an Adult Care Home as a caregiver or resident manager.

WARNING: If you take an Ensuring Quality of Care class, also known as Basic Training, you must pass the Multnomah County Qualifying Test within 90 days of completing the course. Failure to do so will require that you take the class again, a cost of between $250 - $500!

Fire and Life Safety 

Complete the online Fire and Life Safety training for Adult Care Homes, available here: Fire Safety Class (This link is broken, we are working on finding a replacement.) You must complete this training in the 12 month period prior to licensing.
