Steps to enroll in Benefit plans:

Choose your plans.

Enroll in all plans using Workday as part of your Hire - Onboarding process.

You will see a Change Benefits To Do in your Workday inbox.

Step 1: Choose your Medical plan
Medical:

Moda has an extensive in-network provider list, or you pay more money to access out-of-network providers.

Deductible: $1000 and Annual out of pocket maximum: $6,150
-
No charge for in-network Preventive Care services

For other in-network services, you have to meet your $1,000 deductible, and then are responsible for 30% of charges.

For out-of-network services, you pay 50% of the cost after deductible.

Prescription Coverage:

$300 deductible

For 30 day supply: Value/Low Cost prescriptions you pay ≤ $4, all others you pay 30% after deductible.

No Vision coverage.

After you meet your deductible, you are responsible for 20% of the charges for most services.

Vision: $20 copay for annual exam, no coverage for vision hardware.

- Do you have other coverage? You can opt out of medical coverage and receive $125 per month ($62.50 per paycheck).
Step 2: Enroll

Enroll using Workday - this will be part of your Hire - Onboarding process.

Required within 31 days of hire or we will enroll you in a defaulted plan.

An Opt Out Affidavit is required.

Benefits start on the first of the month coincident with or following receipt of forms.
Step 3: Review your other Benefits
TriMet Pass

The County provides a TriMet Annual Pass to use free of charge on all TriMet buses, MAX, Streetcar, Wes, and C-Tran non-express routes.
- Fill out the TriMet agreement form and you will receive your TriMet Hop Fastpass in the mail.
