The mission of IDDSD is to increase an individual’s quality of life by helping them fully access their community through coordinated case management.

Case Management Services

IDDSD Service Coordinators can:

  • Assist in identifying and accessing resources, supports, and services in the community
  • Facilitate and develop individually focused Individual Support Plan (ISP)
  • Coordinate with other State, public, and private agencies regarding services
  • Guide and support the individual to increase overall quality of life, promote independence, and integration, and plan for the future
