Transition Service Coordination Teams, working with the proposed Transition Centers, will link multiple disciplines and agencies (health, housing, vocation, family and community relationships) with community supervision to reduce crime and the risk of re-offense. A Transition Services Advisory Committee will function as part of the Local Public Safety Coordinating Council (LPSCC), linking Multnomah County re-entry efforts to the Governor’s Re-Entry Council.

Budget Range: $1 - 2 million

Status: Not implemented
Although there are no additional funds to develop or implement this program, the proposal was forwarded to Multnomah County's Reentry Council for consideration.
