Scope of Work
REACH’s community clinical linkages strategy focus is to reduce the number of people dying from chronic diseases, through improving connections between healthcare and community health programs, for our African-American and African immigrant/refugee communities. We do this through intentional promotion of appropriate, locally available and culturally specific community health programs and expanding the use of certified Community Health Workers. We also work collaboratively with internal and external partners to identify, recommend, and implement PSE (policy, systems, environmental changes), based on community voice.
