Provider Quality Assurance Records (MHAS2-002): Documents department audits of mental health provider facilities, staff, and practices to conform to Oregon Department of Human Services Addiction and Mental Health Services Division (AMH) requirements. Records may include site reviews, staff background checks, annual conformance plans, certification reports, letter to AMH requesting certificate of approval, corrective plans, variances, contractor application for certificate, correspondence with AMH and providers, and various types of background information required to complete the audit.
