This is a standalone job profile and is not part of a series. Positions in this job profile provide comprehensive community health nursing services to prevent disease and promote community health.

Job description

EXAMPLES OF DUTIES:

  • Conduct community health and population needs assessments, such as education, direct care, community collaboration, and policy development; screen, assess, plan, deliver, and evaluate health interventions for individuals and designated populations.
  • Perform comprehensive health assessments by completing a physical and psychosocial history; identify actual and potential health needs based on medical, behavioral, financial, and social factors; develop comprehensive care plans in collaboration with community partners to establish short- and long-term goals for optimum health and safety of clients.
  • Provide advocacy, counseling, teaching, problem solving, and selection and utilization of resources, while respecting client preferences, social, cultural, spiritual and health beliefs.
  • Coordinate community agency services and teach preventive practices to provide clients quality of life; provide case management and monitor direct care of chronic illnesses.
  • Intervene in crisis situations with high risk families, older adults, and disabled clients; monitor clients for signs and symptoms of physical, emotional, and financial neglect or abuse; teach crisis intervention techniques; implement required nursing procedures and protocols; quickly assess and respond to emergency situations at the individual and community level.
  • Plan, implement, and evaluate case coordination and management; document all activities and maintain client records following applicable rules and regulations; prepare reports.
  • Analyze and review federal, state, and local laws, regulations, policies, and procedures in order to ensure compliance; conduct analysis on best practices and trends, and formulate recommendations.
  • Act as representative on committees, interagency task forces, special projects, and other community outreach activities as assigned.
  • Perform other duties as assigned.

MINIMUM QUALIFICATIONS GUIDELINES:

Any combination of experience and training that would likely provide the required knowledge, skills, and abilities is qualifying. Requirements are determined at the time of recruitment based on responsibilities of the individual position and business needs of the department.

Community Health Nurse

Training and experience for this journey level, professional classification is an associate’s degree in nursing and a license to practice as a Registered Nurse.

Depending upon assignment, candidates may be required to pass a criminal background check, have a valid driver license, and /or additional training, licenses, or certificates.

REQUIRED TRAINING, LICENSES, AND/OR CERTIFICATES:

  • License to practice as a registered nurse as outlined by Oregon Revised Statutes (ORS) 678.021.
  • CPR certification required.
