Job description

EXAMPLES OF DUTIES:

  • Provide information, assistance and referral services including assessment, case planning, education, follow-up, advocacy and triaging; establish initial case files and plans for agencies and staff.
  • Determine appropriate program referrals for clients including protective services, housing providers and community resources; utilize Alliance of Information and Referral Systems (AIRS) classification assessment process, methodology and criteria.
  • Evaluate client needs and eligibility for health and social services; report complaints of suspected abuse and neglect; assist elderly, vulnerable or disabled persons with access to social or health services.
  • Plan, develop, coordinate and implement informational and benefit programs; follow up on key cases to ensure clients are linked to appropriate community resources.
  • Advocate for clients with providers, family members, public and private agencies and others.
  • Assist in the development of brochures, pamphlets, exhibits and displays; maintain inventories of information materials and may participate in public presentations and in-service training sessions.
  • Compile and update resource materials and references; create reports and assist in maintaining electronic documents for distribution and publication; plan, facilitate and participate in meetings, conferences, workshops and training seminars relating to community education and outreach efforts.
  • Perform other duties as assigned.

  • Perform the full range of duties, working independently, applying well-developed subject knowledge, and exercising judgment and initiative.
  • Receive occasional instruction or assistance as new or unusual situations arise.
  • Fully aware of the operating procedures and policies within the work unit.
  • Work is reviewed based on compliance, appropriateness, and effectiveness in meeting goals and deadlines.
  • Receive general supervision from manager or staff.
  • May exercise functional and technical supervision over staff.

MINIMUM QUALIFICATIONS GUIDELINES:

Any combination of experience and training that would likely provide the required knowledge, skills, and abilities is qualifying. Requirements are determined at the time of recruitment based on responsibilities of the individual position and business needs of the department.

Community Information Specialist

Training and experience for journey level, paraprofessional classifications are typically equivalent to an associate's degree and one (1) to four (4) years of experience that demonstrates the ability to perform the duties of the position. (Equivalency 3-6 years of qualifying training and/or experience).

Depending upon assignment, candidates may be required to pass a criminal background check, have a valid driver license, and/or additional training, licenses, or certificates.
