Choice Model Program - Behavioral Health

Helps adults with serious mental illness to live more independently.

Choice connects adults who have complex behavioral health needs with mental health services, so they can live in their community and avoid restrictive care. We work with clients at the Oregon State Hospital, residential facilities and local hospitals. 

Our care coordinators help clients solve problems that make it hard for them to return to their community.

Services

  • Connect to mental health treatment and supports
  • Transitions from hospitals to licensed residential settings
  • Housing support (finding and keeping a placement)
  • Help finding other resources

Who Can Get Services

Enrollment is by referral. Multnomah County adults may be eligible if they:

  • Have severe mental illness
  • Have been in the Oregon State Hospital
  • Live in a licensed residential treatment facility or adult care home
  • Have a history of civil commitment
  • Are unable to aid and assist in their own defense

Participation is voluntary. There are no insurance or income requirements.
