Multnomah County DART is a passport acceptance agent for the U.S. Department of State. If you have questions about a passport you already submitted, contact the U.S. Department of State.
DART is unable to check the status of an application already received by the State Department.
How do I apply for a passport at Multnomah County?
Passport applications are accepted by appointment only Monday-Friday 8:30am -3:30pm. Walk-in service is not available.
Appointments can be made on the scheduling website below.
Appointments are booked on a 30-day rolling window, with new appointments added daily. If you can’t find an appointment that works for you, check back regularly.
Same Day Appointments: We are offering a limited number of same day appointments.
You can call us between the hours of 8:30am-10:30am to check today's availability. These appointments book fast and we encourage you to call early.

Before you schedule an appointment, please read this important information about applying for a Passport at Multnomah County:
Bring a check or money order payable to the US Department of State for the passport application fee. We do not accept credit cards for the application fee.
There is a separate $35 acceptance fee per application, payable to Multnomah County. Payment can be cash, check, money order or credit card.
Failure to bring all necessary application materials will result in forfeiture of your appointment.
Parking at the Multnomah Building is very limited. Please read our parking and transportation suggestions.
Showing up more than 15 minutes late will also result in forfeiture of your appointment.
What information do I need to bring?
Applicants over 16:
Please bring a check or money order made out to the US Department of State. Refer to our Fees page for current amounts.
A completed DS-11 application form, filled out in black ink with no corrections.
Valid Government Photo ID (Drivers License, state-issued ID card, etc.)
Proof of US Citizenship (Certified copy of your birth certificate, expired 5 or 10 year passport, Naturalization certificate, etc.)
A 2 inch by 2 inch passport photo.
Applicants under 16:
All minors must appear in person.
A completed DS-11 application form, filled out in black ink with no corrections.

Both parents must be present.
If one or both parents cannot be present:
The absent parent/guardian needs to complete a notarized Form DS-3053
One parent/guardian can appear with a notarized statement explaining why the other parent/guardian's consent cannot be obtained. Form DS-5525.
Proof of US Citizenship (Birth certificate, expired 5 or 10 year passport, Naturalization certificate, etc.)
A 2 inch by 2 inch passport photo.
Proof of Parental Relationship. The following may be used to show parental relationship
U.S. birth certificate (also evidence of U.S. citizenship)
Consular Report of Birth Abroad or Certification of Birth (also evidence of U.S. citizenship)
Foreign birth certificate
Adoption decree
Divorce/Custody decree
Note: Proof of US Citizenship AND proof of parental relationship MUST be provided with each minor’s application, regardless of how many passports the minor has had previously.
Passport Photos
We take photos of all ages and time for having your photo taken is included with your passport appointment. Passport photos are $10.
Photos of infants and toddlers can be challenging and appointment times will not be adjusted, so please come prepared with ways to make your young ones comfortable. For infants, we strongly recommend that you bring a carseat for their photo.
Only one photo is printed. If you need a second photo for other purposes, such as a visa application, ask the agent at the counter for one at the time of processing.
Appointments are for Passport applications and a photo can be added to that appointment, we are unable to book appointments for photos only.
