Case Management Services
IDDSD Service Coordinators can:
- Assist in identifying and accessing resources, supports, and services in the community
- Facilitate and develop individually focused Individual Support Plan (ISP)
- Coordinate with other State, public, and private agencies regarding services
- Guide and support the individual to increase overall quality of life, promote independence, and integration, and plan for the future
