Become a Provider
Become a Personal Support Worker (PSW)
PSW's provide attendant and relief care services. Clients and families are registered as an Employer of Record (EOR) and utilize a fiscal intermediary for payment. Most PSWs have a connection to the individual they want to work with, but there is also a registry available through the Oregon Home Care Commission. PSW's must have an approved criminal history background check as part of the application process (see the information below to request a background check application). The criminal history background check approval must be renewed every 2 years.

- Complete a PSW Provider Enrollment Application and Agreement (PEAA) form.
PSW's are required to complete new worker orientation and training before being assigned a Medicaid Provider ID number and authorized to work by the Oregon Department of Human Services. Orientation and training is provided by Carewell SEIU 503. Orientation and training information is available at: Carewell SEIU 503 Training website
Become a Child Foster Care Provider
Children co-served by the I/DD and Child Welfare programs and children living in their family homes or other settings are eligible for placement into I/DD certified foster homes. Some homes are certified to support children with complex medical needs and all homes are expected to apply Positive Behavior Support and Trauma Informed approaches to the care children receive. All providers receiving funding from I/DD are required to log into eXPRS and submit claims for provider payment.

Become an Adult Foster Care Provider
Adults and their families receiving I/DD services may choose to be placed in foster care to live a more independent life with the supports they need. The Adult Care Home Program with Multnomah County is responsible for the licensing and training of all foster care providers who provide care to individuals from multiple systems within Multnomah County, including I/DD.
Providers not licensed by Multnomah County
The following providers are licensed directly by the State of Oregon with Oregon Developmental Disability Services (ODDS):
- Group Home
- Supported Living
- Employment
- Behavior Consultants
- Employment Support Specialists
- Transportation
Background Checks for I/DD Service Providers
I/DD service providers must have a Criminal History Background Check approved by the Oregon Department of Human Resources/Oregon Health Authority Background Check Unit (BCU). Background check applications are submitted by designated Multnomah County I/DD staff to BCU for the following types of service providers serving I/DD clients:
- Personal Support Workers (PSW)
- Behavior Consultants
- Employment Support Specialists
- I/DD Direct Nursing Service Providers
The BCU background check application process is entirely online. There are no paper forms used.
Providers must have a valid email address. The background check system will email you a link to complete the application online. Multnomah County I/DD will send notices, instructions and information about your background check application to you by email.
Valid government-issued photo ID is also required:
- State-issued driver's license
- State-issued ID card
- U.S. Armed Forces ID card
- U.S. Passport
- Permanent Resident ID

You will receive any additional information and instructions during that meeting and by email afterward.
