DDSD Services Coordinators assist adult clients with developmental disabilities in getting their needs met. They are responsible for monitoring a client's services in residential sites and those clients who have in-home support plans. Specifically, Services Coordinators work to ensure that the Individual Support Plan is being followed, there are no health and safety issues, medical issues are being addressed and a person's money is being used appropriately.
The DDSD Children and Young Adults Program provides case management services to children with an intellectual or developmental disability. The children we serve live in a variety of settings. We serve children who live at home with their families, in funded care with non-relative families, and some who live in residential settings like Group Homes.
