Multnomah County provides voluntary, individually focused care coordination and planning to children and adults with Intellectual and/or Developmental Disabilities.

Anyone can make a referral with a client or legal guardian's consent. To make a referral:

How do I apply?

  1. Submit a referral / Complete este formulario de referencia

  2. Our intake coordinator will contact you to schedule an intake appointment with an Eligibility Specialist.

  3. You will meet with an Eligibility Specialist to complete an application packet and learn about services and timelines. The meeting can occur over the phone, video meeting or in person.  

An intake appointment is not required. You can complete and return an application packet via the following methods:

Who is eligible?

We serve individuals diagnosed with intellectual and developmental disabilities per OAR 411-320-0080. Examples of potentially qualifying conditions are: Intellectual Disability, Down Syndrome, Autism, Cerebral Palsy, Epilepsy, Traumatic Brain Injury, Fetal Alcohol/Drug Syndrome.

  • The condition must be expected to last a lifetime.

  • The condition must significantly impact at least two everyday living skills. Examples include self care, communication, learning, socialization, mobility and self-direction.

  • The onset of the intellectual disability must have occurred prior to age 18. The onset of the developmental disability must have occurred prior to age 22.

Can I appeal if I am found not eligible?

You have the right to challenge an eligibility decision by requesting a contested case hearing within 90 calendar days of the Notification of Planned Action date. To request a hearing:

  1. Fill out an Administrative Hearing Request Form ( SDS 0443DD ) and send it to:

  2. Fax an Administrative Hearing Request Form ( SDS 0443DD ) to 503-373-7274
  3. Make a verbal request for a hearing by speaking directly with the Eligibility Specialist you have been working with.

A Medical Statement may be used to assist or determine eligibility for services. Return completed forms to idd@multco.us
