Case Management Services for Adults

IDDSD Services Coordinators assist adult clients with intellectual or developmental disabilities in getting their needs met. They are responsible for monitoring a client's services in residential sites and those clients who have in-home support plans. Specifically, Services Coordinators work to ensure that the Individual Support Plan is being followed, there are no health and safety issues, medical issues are being addressed and a person's money is being used appropriately.

Case Management Services for Children and Young Adults

The IDDSD Children and Young Adults Program provides case management services to children with an intellectual or developmental disability. The children we serve live in a variety of settings. We serve children who live at home with their families, in funded care with non-relative families, and some who live in residential settings like Group Homes.
