Case Management Services for Adults
The IDDSD Adult Case Management Program provides case management services to adults with an intellectual or developmental disability who reside in a variety of settings, including individuals living independently in their own homes, people living with their family or friends, and adults receiving residential services.
If you are 18 years old or older and do not access residential services, you may be able to choose who coordinates your services. Case Management entity choices are:
- A Services Coordinator with Multnomah County I/DD or
- A Personal Agent from a Support Services Brokerage

Case Management Services for Children and Young Adults
The IDDSD Children and Young Adults Program provides case management services to children with an intellectual or developmental disability. The children we serve live in a variety of settings. We serve children who live at home with their families, in funded care with non-relative families, and some who live in residential settings like Group Homes.
