Inpatient Mental Health Discharge Services (MITT)

Connects people with services and care to get well after they leave the hospital.

The Multnomah Intensive Transition Team (MITT) helps adults get ready to leave a mental health treatment center. We provide short-term support that connects clients with long-term behavioral health and specialized care after discharge.

The goal is to help them get well and avoid having to return to the hospital. 

For clients on Medicaid through Health Share who don't already have mental health services.

Who Can Get Services

Any adult who has been admitted to a psychiatric hospital, and they:

  • Live in Multnomah County
  • Have Medicaid through HealthShare of Oregon
  • Don’t have an outpatient mental health provider

Referrals come directly from Health Share.

How It Works

We meet clients in the hospital to offer services, see what they need and develop a service plan. Together, we’ll work with hospital social workers to create a hospital discharge plan.

After they leave the hospital, we’ll follow up with the client to make sure they're doing well. We'll help them overcome barriers that make it hard to get services.
