We are in the fieldwork phase of this audit. 

Audit Objectives: 

  • Assess whether interdisciplinary communication among Corrections Health staff, as well as between Corrections Health staff and Multnomah County Sheriff's Office and Department of Community Justice staff, addresses the mental health needs of individuals in custody while mitigating safety risks.
  • Evaluate whether incident reporting by staff and grievance processes for individuals in custody are accessible, transparent, impactful, and supported by workplace culture.
  • Evaluate the implementation of suicide prevention practices and their compliance with national standards, including screening, direct observation, and post-crisis follow-up care.
  • Assess whether quality management and executive oversight identify and mitigate systemic risks to patient safety, including within the methadone program and restrictive housing.

Audit Scope: 

We will focus on Corrections Health services from fiscal year 2025 through August 31, 2026, depending on the specific audit objective and data availability. We may examine Department of Community Justice and Multnomah County Sheriff’s Office data that pertains to audit objectives.
