The Community Partnerships & Capacity Building program partners with culturally-specific communities to better address their public health priorities. Work centers on community health workers and cultural organizations—they hold the wisdom and power to create long-term, meaningful change.
We also work to improve how our public health programs partner with the community. And to support the Health Department’s culturally-specific programming.
When we support cultural and community-driven initiatives, we help to undo systemic injustice and racism. So that everyone in Multnomah County can be healthy.

Community Health Improvement Plan (CHIP)
A CHIP describes the priorities, goals, solutions, and resources for health improvement. Our CHIP was informed by community health assessment findings and community input. It centers the experiences of communities most affected by health disparities and the solutions they recommend.
