What is Coordinated Access?
Coordinated Access in Multnomah County
The intent of Coordinated Access is to provide streamlined and equitable access to shelter and housing interventions for people with disabilities experiencing homelessness in Multnomah County. Regardless of where someone first seeks services, access is based on vulnerability, eligibility, and choice. This system creates a single and more equitable, intake, assessment and referral process, and allows us to prioritize limited resources for the most vulnerable households in our community. If you are disabled and homeless and would like to be assessed and referred to the Coordinated Access wait list, reach out to the entities below and request a Coordinated Access assessment:

Adults: Please contact the Coordinated Housing Access Team (CHAT) Hotline at 844-765-9384. You can also complete an online pre-screening by filling out this form.For culturally specific assessments for the BIPOC community, you can call/email the BIPOC CHAT Line at 503-280-2600 x 654 or housingreferrals@ulpdx.org.
Veterans: Call the Veteran Services hotline at 855-425-5544.

Shelter Plus Care
Multnomah County Intellectual and Developmental Disabilities has a Shelter Plus Care grant, which is a Permanent Supportive Housing resource aligned with the Coordinated Access system. Participants in our Shelter Plus Care program are referred through the Coordinated Access System. The grant is full and new participants can only be referred when an existing participant leaves the program. New referrals come through the Coordinated Access wait list system.
