Intensive Care Coordination provides consultation and guidance to providers and Multnomah County residents who are seeking information about and/or access to children’s mental health services. A care coordinator is offered to all youth receiving treatment in the higher levels of mental health care. The mental health service array for youth is available to all members based on the Health Share of Oregon Practice Guidelines.
To Request Services
If you are a provider or resident seeking to refer a youth to Home Based Stabilization (HBS), Psychiatric Day Treatment (PDTS), or Psychiatric Residential Treatment Service (PRTS), complete the following:
- Authorization to exchange and disclose health information
- Mental health assessment completed within the last 60 days along with most recent progress notes
Fax completed forms to 503-988-3328 or scan and submit via e-mail to firstname.lastname@example.org. For assistance with making a referral, please call our Referral Coordinators at 503-988-4161. You should expect a response within one business day from our Referral Coordinators.
