Individual Support Plan
Each child served has an individually tailored plan of care called an Individual Support Plan developed by the child's network of support. It defines how the Service's Coordinator will support them throughout the year. The I/DD Services Coordinator works closely with the child and their family/network of support to help them progress toward their identified goals comprised of educational aspirations, community integration goals, and plans for transitioning into adulthood.

Case Management services aim to assist children and their family in accessing community resources to promote community inclusion.
Services through case management often include connection to resources like medical, dental, social security/supplemental security income, housing, clothing, food, community engagement activities, adaptive equipment, and other supportive services.
Family specific services as funding is available often include parent and family training, behavioral consultation, funded in-home support, respite care, counseling, and other supports that meet occupational, physical, and communication therapeutic support needs.
Kplan Funding for Services children who are eligible, access KPlan funded services in the family home based on a needs assessment which determines the type and frequency of a variety of supports. They include, but are not limited to, attendant care, relief care, skills training, behavior consultation, assistive technology, and assistive devices. Through waiver services, Family Training is also available if indicated in the needs assessment.
Foster care - Children co-served by the I/DD and Child Welfare programs and children living in their family homes or other settings are eligible for placement into I/DD certified foster homes. Some homes are certified to support children with complex medical needs and all homes are expected to apply Positive Behavior Support and Trauma Informed approaches to the care children receive.
This unit collaborates with public school systems, Oregon Children, Adults and Families Division (CAF), the Social Security Administration, Family-to-Family Networks like FACT, and other community programs. The Intellectual and Developmental Disabilities Children and Young Adults Program provides case management services to children who meet eligibility criteria for having an intellectual or developmental disability. All services and funding available are based on criteria found in the Oregon Administrative Rules (OARs) for Community Development Disabilities Programs and Case Management Services for Individuals with Intellectual or Developmental Disabilities.
