After Hours Triage Notes (DOH27-001): Records documenting the assessment and advice to or referral of people who contact Multnomah County's Health Department outside of it's operating hours. This service is provided by a contractor and the results are forwarded to the call center. Records show date and time called, caller's name, needed services/advice, and resolution.

Call Schedules (DOH27-002): Records documenting appointment schedules for clients at various clinics and is used to make reminder calls the night before the patient appointments. Record is a printout of the health information system scheduling database showing clinic and client names and the time and date of the appointment.

Records are only maintained in hard copy for emergency room calls; all others are maintained in the Health Information System. Records show date and time called, caller's name, needed services/advice, and resolution.

Well Newborn Faxes (DOH27-005): Documents faxes sent from the call center to the patient's prospective clinic site. These are composed of a cover sheet, HIS system printout confirming the appointment time and date, and OHSU's well newborn summary discharge notes.
