Child Custody Evaluation Studies (JSD11-002):

Documents the written professional opinion concerning custody and parenting time for families that have not been able to come to agreement through mediation. Records may include recommendation to the court, modifications to existing custody studies, information forms, intake forms, psychological evaluations of family members, social worker reports, phone messages, work notes, information release forms, related correspondence, and supporting documentation (report cards, wills, child support documentation, restraining orders, etc), recommendation to the court, modifications to existing custody studies, information form, and intake form.
Retention: Retain until youngest person in case record reaches age 21. (MOAR 166-150-0130(9))

Coaching Files (JSD11-013):

Documents court-ordered and voluntary co-parent coaching. Records may include agreements, case notes, certificates of completion, closing memos, court orders, and other related and supporting documentation.
Retention: Retain 7 years after completion of sessions. (OAR 877-030-0100(3) and OAR 833-075-0070(1a))

Conciliation Case Files (JSD11-011):

Documents the provision of marriage counseling to couples considering divorce or separation. Counseling can either be informal marriage counseling or formal court conciliation counseling. Case records may include intake form, case face sheet, conciliation petition to the court, minute orders, counselor notes, and related correspondence. Key information includes file number, case name, counselor name, clients' personal and family history, appointment dates, service type and date, and final disposition.
Retention: Retain 6 years after case closed. (MOAR 166-150-0130(2))

Mediation and Custody Case Index (JSD11-003):

Used to access summary case information and to cross reference cases by both parents' names. Index shows case name, case number, date opened, and date closed.
Retention: Database: retain 2 years after removed from use; Cards: retain 2 years after entered and verified in database. (2007-0008/002)

Mediation Case Files (JSD11-001):

Documents the provision of assistance in planning custody or visitation with minor children. Case records may include intake form, case face sheet, order regarding custody and visitation (after mediation), mediation agreements, work notes, and related correspondence. Key information includes file number, case name, counselor name, appointment dates, service type and date, and final disposition.
Retention: Retain until youngest person in case record reaches age 21. (MOAR 166-150-0130(9))

Mediation Orientation Records, Parent Education Certificates, and Parent Education Orientation Attendance Records (JSD11-006):

Documents that both parties have attended mediation orientation sessions required by ORS 107.755 and fulfilled court-ordered education requirements. Records include rosters and certifications and may show petitioner and respondent names, case numbers, attendance and fee information. Note: This supersedes JSD11-004 and JSD11-005. Use JSD11-006 for all mediation and education records.
