Accounts Payable Records (DOHGEN-001): Records documenting payment of division bills for general accounts excluding grants. Includes reports, invoices, statements, vouchers, purchase orders, payment authorizations, receipt records, canceled checks or warrants, and similar records. This information is maintained primarily in SAP with a variety of paper and electronic supporting documents.

Accounts Receivable Records (DOHGEN-002): Records documenting revenues owed to the division to be credited to general accounts excluding grants. Also documents billing and collection of moneys. May include reports, receipts, invoices, awards, logs, lists, summaries, statements, and similar records. Information often includes, receipt amount, date, invoice number, name, account number, account balance, adjustments, and similar data. This information is maintained primarily in SAP with a variety of paper and electronic supporting documents.

Annual Statistical Reports (DOHGEN-003): Documents program activities for research and reference. Compiled and maintained for internal reference as well as to comply with division requirements. Reports show type of visit, services provided, specialty referrals and procedures, cancellations, and historical comparisons.

Budget Preparation Records (DOHGEN-004): Records documenting preparation of division budget requests to be included in the department budget. May include staff reports, budget instructions, worksheets, surveys, allotment reports, spending plans, contingency plans, budget proposals, financial forecasting reports, and similar records. Much of this information is maintained in electronic form in spreadsheets and similar documents.

Calendars and Scheduling Records (DOHGEN-005): Records documenting and facilitating routine planning, scheduling, and similar actions related to meetings, appointments, trips, visits, and other activities. Includes calendars, electronic (including handheld) organizers, appointment books, notes, telephone messages, diaries, and similar records.

Child Abuse Reports (DOHGEN-006): Records document suspected child abuse reported by program employees. Records may include but are not limited to notes and observations of the child, record of contact with the State Office for Services to Children and Families or law enforcement agency, and related documentation.

Client Incident Reports (DOHGEN-039): Documents any incident involving a client, patient or visitor to an MCHD worksite. Reports show client, patient or visitor name, case number, location of incident, date and time of incident, description of the incident, action taken by staff, and dated signature of reporting staff member.

Client Satisfaction Surveys (DOHGEN-007): The survey indicates client satisfaction with program services. Includes surveys and summary reports detailing various services provided and customer satisfaction with them.. Survey information includes information about the program, a description of services provided, number of persons served, and a breakdown of client responses.

Communication Logs (DOHGEN-043): Logs document communications made or received through a variety of electronic devices, including but not limited to telephone, smart phone, facsimile (fax), radio, computer-aided dispatch, pager, and teletype, AND are not otherwise specified in the county's retention schedule. Logged information may include time, date and disposition of communication, name of caller, number called or received, and action taken. Includes forms that document the progress taken to contact clients regarding appointments.

Community Surveys (DOHGEN-040): Surveys that document a specific or general community's opinion, feedback or experience with a set topic area. These surveys can collect information about the behavior of specific communities or a specific community's attitudes, needs and opinions around various issues or initiatives. This information can be used to create targeted services and/or outreach campaigns, help make related programmatic, policy and funding decisions, and provide information on new and emerging trends.

Confidentiality Agreements - Contractors and Visitors (DOHGEN-042): Records document contractor and visitor compliance with policies regarding protecting client privacy while on site. These are the agreements signed by individuals who are not covered by an existing business associate agreement.
Retention: Retain 10 years. (OAR 410-141-3520(10); 45 CFR § 164.316(b)(2)(i))

Contract Administration Records (DOHGEN-008): Documents the management of contractor activities and payments for all division contracts. Records include expense reports, copies of payment vouchers (including payment memos and invoices), contract approvals, contract amendments, contract review checklist, contract routing form, copy of contract, contractor information (bankruptcies, lawsuits, etc), and related correspondence. Records may also be included in SAP and in a variety of supporting electronic and paper documents.

Correspondence: Records that: 1. document communications created or received by an agency AND 2. directly relate to an agency program or agency administration AND 3. are not otherwise specified in the Multnomah County Retention Schedules. Records may include but are not limited to letters, memoranda, notes and electronic messages that communicate formal approvals, directions for action, and information about contracts, purchases, grants, personnel and particular projects or programs.

Daily Cash Logs (DOHGEN-013): Series documents cash received for licenses issued and the daily deposit of that cash. Log shows date, number of licenses issued, cash collected, amount deposited, and date deposited. Includes food handler and vital statistics cash logs.

Employee Time Records (DOHGEN-014): Records documenting hours worked, leave hours accrued, and leave hours taken by department employees. Information usually includes employee name and social security number, hours worked, type and number of leave hours taken, total hours, dates, and related data. Includes applications for leave. This information is maintained primarily in SAP with a variety of paper and electronic supporting documents.

Encounter Records (DOHGEN-015): Documents client information and services provided at each clinic visit for entry into the Health Information System. Includes encounter forms indicating client demographic information, and check-off boxes for data entry indicating services provided during the particular site visit.

Equipment Maintenance Records (DOHGEN-016): Records may include purchase orders, lease agreements, warranties, vendor statements, service contracts, charge call bills, fax activity reports, property disposition requests, invoices for equipment repair, purchase request forms, and memoranda.

Grant Records (DOHGEN-017(a) and DOHGEN-017(b)): Records document the application for and administration of grants in which the department is the recipient or administrator. Records may include but are not limited to applications including project proposals, summaries, objectives, activities, budgets, exhibits, and award notification; grant evaluation records and recommendations concerning grant applications; grant administration records including progress reports, budgets, project objectives, proposals, and summaries; records documenting allocation of funds; contracts; records monitoring project plans and measuring achievement; equipment inventories; financial reports, accounting records, audit reports, expenditure reports, and related correspondence and documentation.
Retention: DOHGEN-017(a) Retain final reports from significant grants to the county permanently; DOHGEN-017(b) Retain other grant records: 3 years after annual or final expenditure report submitted and approved or, as specified in agreement, whichever is longer. (MOAR 166-150-0110(17)

Incident Reports (DOHGEN-018): Series used to report non-injury safety incidents and injuries to non-employees. Records may include incident reports, police reports, investigation records, and related correspondence.

Key and Keycard Records (DOHGEN-019): Records document the issuance of keys and keycards to staff to enable access to buildings and sites. Records may include but are not limited to key inventories, key issue forms, key replacement records, and key disposal records.

Legislative Tracking Records (DOHGEN-020): Series used to monitor legislation that may have an impact on the department's current operations or policies. Records include concept statements, proposals, bill logs, fiscal/organizational impact analysis papers, copies of bills, testimony summaries, committee reports, agendas, and correspondence.

Material Safety Data Sheets (DOHGEN-037): Series documents all hazardous chemicals used and held by an agency. Records include hazardous materials safety sheets, safety instructions, and emergency instructions.
Retention: Retain until superseded. (MOAR 166-150-0200(11))

Monthly Clinic Reports (DOHGEN-021): Monthly reports contain statistical data about client visits to health providers. The information is used for a variety of research and reporting purposes. Report titles and numbers change over time, but generally cover the number of medical and dental visits by service location, program area, and provider; a comparison of actual versus targeted visits; and walkouts without service.

Personnel Accident Incident Reports (DOHGEN-038): Series used to report employee accidents to program supervisors. Records may include SAIF accident reports, accident reports, occupational injury report and investigation, and employee identification and physical assessment form.
Retention: Retain 10 years after case closed. (OAR 166-150-0200(13))

Petty Cash Records (DOHGEN-022): Documents authenticating petty cash purchases and requesting reimbursements from the department petty cash fund. Records include receipts, transaction statements, petty cash form, and memo requesting payment. The primary transactional record is maintained in SAP, with supporting documentation, including electronic information, maintained separately.

Policy and Procedures Records (DOHGEN-023(a) and DOHGEN-023(b)): Series documents review, assessment, development, and authorization of the department's formal policies and procedures. Records may include authorizing bulletins and advisories, mission and goal statements, manuals, and final policy statements and directives. Used for new employee orientation and for ongoing reference. Also useful in establishing past policies or procedures in liability cases, personnel disputes, and other instances. This series also includes routine documentation and basic clerical instructional procedures covering such subjects as formatting letters, data entry, telephone etiquette, and others. Project Records (DOHGEN-025(a) and DOHGEN-025(b)): Records documenting various long- and short-term projects for which the department has primary recordkeeping responsibility. Records vary from project to project, but usually include research papers and reports, resolutions, project reporting, background material, project plans and goals, correspondence, and similar material.

Publications (DOHGEN-026): Published records produced by or for the department or any of its programs and made available to the public. Includes newsletters, pamphlets, brochures, leaflets, reports, studies, proposals, and similar published records. Does not include publications received from federal, state, private, or other sources - these publications and extra copies of agency produced publications should be retained as needed. For publications maintained in electronic format, please contact the County Records Management Program for preservation assistance.

Purchasing Records (DOHGEN-027): Records documenting orders, authorizations, and evidence of receipt of the purchase of goods and services by the department and its programs. Includes purchase orders and requests, purchase authorizations, requisitions, contract release orders, material and cost specifications, central stores or printing orders, telephone service orders, and similar records. The primary transactional record is maintained in SAP, with supporting documentation, including electronic information, maintained separately.

Quality Assurance Records (DOHGEN-041): Records documenting internal quality assurance activities, such as compliance audits and chart reviews. Records may include but are not limited to evaluations, performance reports, improvement recommendations, and related documentation.

Reports and Studies (DOHGEN-028(a) and DOHGEN-028(b)): Records document special reports or studies conducted on the department's programs, services, or projects, compiled by division personnel, or by consultants under contract. Includes final report distributed either internally or to other entities and the work papers used to compile the report or study. Safety Program Records (DOHGEN-029): Records document the department's program to promote a safe work environment for its employees. Seminar and Conference Records, External (DOHGEN-031): Records documenting activities of seminars, conferences, workshops, conventions, and similar gatherings not sponsored by the department but attended by department officials or personnel. May include staff reports, instructional materials, recommendations, related correspondence and memoranda, and similar records.

Seminar and Conference Records, Internal (DOHGEN-030): Records documenting the design and implementation of department sponsored seminars, conferences, workshops, conventions, and similar gatherings. Often includes class descriptions, instructional materials, course outlines, enrollment and attendance records, reports, speeches, planning documentation, and related records.

Staff Meeting Records (DOHGEN-032): Records documenting routine department staff meetings which are not subject to Oregon's Public Meetings Law (ORS 192.610 to 192.710). These meetings deal with tasks and actions within existing policies and procedures. Records may include minutes, notes, reports, and related items. Some records may merit inclusion in other record series with longer retention periods if the subject matter of the meeting adds significant information to that series.

Strategic Planning Records (DOHGEN-033(a) and DOHGEN-033(b)): Series documents long-range plans and the development of the department's, or its subdivision's, work objectives. Records include strategic plans, mission statements, preliminary drafts, work notes, and related correspondence.

Technical Manuals, Specifications, and Warranties (DOHGEN-034(a) and DOHGEN-034(b)): Owners manuals and warranties for agency-owned vehicles and equipment. Manuals often include specifications, operating instructions, and safety information. Warranties include terms of coverage for repair or replacement of equipment.

Translation Service Requests (DOHGEN-035): Records documenting program requests for translation services. Record includes date, language(s) to use, text to be translated, instructions, and supporting documentation.

Travel Expense Records (DOHGEN-036): Records documenting requests, authorizations, reimbursements, and other actions related to employee travel. Includes expense reports and receipts, vouchers, requests, authorizations, and related documents. Retention applies to private vehicle usage as well. Information often includes estimated costs, prepayments, final costs, destination, method of transportation, travel dates, approval signatures, and related data.
