Series Title (Agency Series #): Description, Retention (Legal Citation)

Correspondence: Records that: 1. document communications created or received by an agency AND 2. directly relate to an agency program or agency administration AND 3. are not otherwise specified in the Multnomah County Retention Schedules. Records may include but are not limited to letters, memoranda, notes and electronic messages that communicate formal approvals, directions for action, and information about contracts, purchases, grants, personnel and particular projects or programs.
Retention: File with the associated program or administrative records. Communications not meeting the above criteria do not need to be filed and may be retained as needed. (OAR 166-150-0005(10)) Updated 01/08/2018

Education and Training Program Records (DOH35-002): Records related to the design and implementation of training programs provided by the School and Community Dental Program May include class descriptions, instructor certifications, planning documentation, instructional material, course outlines, class enrollment and attendance records, brochures, and related significant records.

Sealant Treatment Records (DOH35-003): Individual case records documenting the application of dental sealant to school children in selected grade and middle schools. Records include class lists, treatment charts, permission slips, and related records. Information includes school name, date of treatment, type of treatment names of students treated, and related information.

Volunteer Records (DOH35-004): Records documenting the activities and administration of volunteers. Records include volunteer sign-in book, volunteer application, hours totals, and orientation and training documentation.
