Laboratory Test Records (DOH39-001(a) and DOH39-001(b)): Records documenting lab test conducted for clinics and programs within the Health Department. Records include a copy of the test result, any supporting documentation, and in the case of pap smears, physical test specimens. Information includes patient name, date of test, test results, organization performing test, and related information. Records may be duplicated in the patient's chart and in records held by the reference lab.
