Immunizations are required by state law for children and students in attendance at public and private schools, preschools, childcare facilities and Head Start programs in Oregon. Nearly every facility that provides care for a child outside the home requires immunizations or a medical or non-medical exemption to stay enrolled.

If you do not meet the state requirements for immunization you will receive an exclusion notice. This is a letter stating that your children must be immunized or will not be admitted to school starting on the school exclusion date.

Parents

  • Review the immunization requirements (below) and have your child immunized.
  • Fill out a Certificate of Immunization Status Form with the immunization dates for your child. You may also request this form from your child’s school or childcare facility.
  • Sign the form and turn it in to your child’s school.

If you receive an exclusion notice, contact your medical provider to have your child immunized. Children who are not up-to-date on their immunizations by February 18, 2015 will be excluded from school until their immunizations are up-to-date.

School and Childcare Facilities


Important Dates

First day of school or at a certified child care facility - All children must show proof of one dose of all required immunizations. Exception: Students who transfer from another school in the United States have 30 days to have their immunization records transferred to the new school.

January 14, 2015 - Deadline for schools and certified child care facilities to report to Multnomah County Health Department.

February 4, 2015 - Multnomah County Health Department mails exclusion orders to children's families who are not up-to-date on their immunizations.

February 18, 2015 - Exclusion day. All children who are not up-to-date or complete on their immunizations will be excluded from their school or certified child care facility.


Immunization Requirements 2014-2015

Required doses vary by a child’s age and how long ago they were vaccinated. Please check with your child’s school, certified child care facility, healthcare provider or Multnomah County Health Department for details.

Preschool, Child Care or Head Start

  • 4 doses - Diphtheria/Tetanus/Pertussis (DTaP)
  • 3 doses - Polio
  • 1 dose - Varicella (chickenpox)
  • 1 dose  - Measles/Mumps/Rubella (MMR)
  • 3 doses - Hepatitis B
  • 2 doses - Hepatitis A
  • 3 or 4 doses - Hib

Kindergarten plus grades 1-6

  • 5 doses - Diphtheria/Tetanus/Pertussis (DTaP)
  • 4 doses - Polio
  • 1 dose - Varicella (chickenpox)
  • 2 doses - Measles
  • 1 dose - Mumps
  • 1 dose - Rubella
  • 3 doses - Hepatitis B
  • 2 doses - Hepatitis A

Grades 7-12

  • 5 doses - Diphtheria/Tetanus/Pertussis (DTaP)
  • 1 dose - Tdap
  • 4 doses - Polio
  • 1 dose - Varicella (chickenpox)
  • 2 doses - Measles
  • 1 dose - Mumps
  • 1 dose - Rubella
