To qualify for WIC you must:
  • Live in Oregon
  • Be a woman who is pregnant or has just given birth, an infant or a child under 5 years old
  • Meet income guidelines
  • Have a nutritional need or risk

Dads, guardians and foster parents can apply for WIC for their children.

You do not have to be a U.S. citizen to get WIC services.

What to Bring to Your First Appointment

1. Identification, such as:

  • Oregon driver's license
  • State ID card, a passport
  • Work or school ID
  • Oregon Health Plan medical ID card
  • Current WIC ID card

2. Proof of income, such as:

  • Oregon Health Plan Healthcare.gov approval letter
  • SNAP (food stamps) award letter*
  • Temporary Assistance to Needy Families (TANF) Notice of Approval, or most recent Change Notice letter
  • 1 month of most recent pay stubs
  • Unemployment benefit stub
  • Most recent W2 forms or income tax return
  • Foster child/parent placement letter
  • Signed letter from employer stating gross earnings
  • Food Distribution Program on Indian Reservations (FDPIR)

*We are unable to accept the Electronic Benefits Card (Oregon Trail Card) as proof.

Are you eligible for the Oregon Health Plan, TANF, food stamps or FDPIR? Then you're automatically income-eligible for WIC. Proof is required.

3. Proof of residence, such as:

  • Oregon driver's license
  • State ID card
  • Current utility bill
  • Bank statement
  • Rent receipt

4. WIC ID card, if you already have one.

If the appointment is for your child, also bring:

  • Proof of identity, such as a birth certificate, an immunization record or an Oregon Health Plan medical ID card
  • Their immunization record, if you have it

When you make the appointment, ask if you should bring anything else.

What to Expect

At your first appointment we'll find out if you're eligible and ask to see proof of income, identification and address. We'll also check your health.

  • You and your child will be weighed and measured.
  • Your child's growth and development will be discussed with you.
  • You will receive a hemoglobin or iron screening.
  • Your eating habits will be reviewed.
  • Your medical/nutritional history will be taken.
  • Together we'll create a family health and nutrition plan
  • You'll get an electronic benefits card to buy WIC-approved foods at your local grocery store.
