Unum will no longer accept new Multnomah County enrollments beginning 2/1/2026. Employees with current Unum coverage have the opportunity to continue their coverage by paying premiums directly to Unum. Continuation forms were mailed to these employees on 01/07/26.
Trustmark will be our new Long Term Care provider, offering life insurance with long term care - you elect a life insurance amount up to $300,000, and can receive 4% of the benefit each month for up to 50 months of professional long term care or 2% of the benefit each month for up to 100 months of family care. There will be an initial enrollment period from 2/2/26-2/27/26, for coverage to start 4/1/26. Information sessions are scheduled for:
- Tuesday, February 3, 4:00-5:00pm
- Thursday, February 12, 10:00-11:00am
- Wednesday, February 25, 12:00-1:00pm

What is Long Term Care (LTC)?
Long Term Care is the type of care you may need if you couldn't independently perform the basic activities of daily living: bathing, dressing, using the toilet, transferring from one location to another, continence and eating, or if you suffered severe cognitive impairment from a condition such as Alzheimer's disease.
Eligibility
Long term care coverage is an option for employees and their eligible family members (including domestic partners) in the following groups:
- AFSCME Local 88
- Physicians, sub-Local 88-2
- Pharmacists, sub-Local 88-4
- Dentists, sub-Local 88-5
- MCCDA
- ONA
- Non-represented
Guarantee Issue
During a new employee's first 31 days of hire, the plan provides the employee with Guarantee issue coverage levels in one of four different plans, with up to $6,000.00* monthly benefit for up to six years of payments. Guarantee issue means that the application will be approved up to specific amounts without needing to provide medical information and medical history. Guarantee issue only applies to employee applicants, not family members.
Coverage amounts requested in excess of the Guarantee Issue allowances will be subject to review of medical history by Unum. In this instance, each applicant must also complete a Medical History Form.
Enrollment Outside of the Guarantee Issue Period
All family member applications and employee applications submitted after the 31 day guarantee issue period will go through the regular application and review process, which includes providing medical history to UNUM. Unum has the right to approve or deny these applications. Coverage is not established until approved by UNUM.
Coverage Amounts and Premiums
The minimum amount of coverage is $2,000 and the maximum is $9,000. Premium costs vary based on age, selected plan, and amount of requested coverage. Employee and Spouse/Domestic Partner coverage can be paid through payroll deduction.
