Moda Provider Search - You can search for Moda's providers as a guest by selecting the "Connexus" network for medical and the "PPO Delta" or "Dental Premier" networks for dental. After your plan start date, you can also search by using your subscriber ID# or search in your Member Dashboard. Note: For employees working outside of the Moda service area, you will use the "Aetna PPO" network.
