Series Title (Agency Series #): Description, Retention (Legal Citation)

Food Handler Examinations (DOH16-001): Record of test taken by licensee to obtain a food handler license. Tests can be taken in written format at Multnomah County Health Department, at various site locations and by online testing. If completed with a passing grade, the test results are entered into or downloaded to the Food Handler Database and a license is immediately given to or printed by the licensee. If completed with a failing grade, a no pass notation is entered in the database. Information includes licensee's name, test questions and answers, and score. Only the final score is entered in the database.
Retention: Retain until entered and verified in Food Handler Database. (2009-0001/007)

Food Handler License-Holder Report (DOH16-002): Documents people holding current food handler licenses. Report is generated by the Food Handler Tracking System and shows person's name, license number, date license expires, and related information.
Retention: Retain 3 years. (2009-0001/008)

Food Handler's Certification Record (DOH16-003): Document's food handler licenses issued and entered in tracking system. Records include a food handler's certification record for each license issued and office processing forms or computer generated reports for each daily batch of certification records for each location. Information includes license number; licensee's name, date of birth, and zip code; date batch issued and processed, total fees received, date entered in tracking system, and date entered into Excel financial system.
