Access the application to Exempt a Residence Address from Disclosure as a Public Record (SEL 550)

A voter may request that a county elections official not disclose the residence address of the voter. To do so, the voter must demonstrate to the satisfaction of the county elections official that the personal safety of the voter, or the personal safety of a family member residing with the voter is in danger if the residence address remains available for public inspection.  If approved, the county elections official shall not disclose that information except in compliance with a court order, a request by a law enforcement agency, or with the consent of the voter.

Complete the Confidential Voter Application Form. The exemption request needs to be in writing, signed by the voter, and include:

  • (a) The name of the voter requesting exemption;

  • (b) A non-exempt mailing address for the voter; and

  • (c) Evidence sufficient to establish to the satisfaction of the county elections official that disclosure of the voter's residence address would constitute a danger to the personal safety of the voter, or of a family member residing with the voter.

Complete an Oregon Voter Registration Card.

Note: Military and Out-of-Country Voters should complete the Voter Registration and Absentee Ballot Request (FPCA) instead of the Oregon Voter Registration Card.

Submit the completed confidential voter application form with the required evidence, and the completed voter registration card to:

Once approved, confidential voters are required to submit a newly completed application when submitting a change in voter registration, such as a change in residential and/or mailing address.
