Community Health Hubs
Community Health Hubs are places in Multnomah County where people can receive free, Black and African-focused, culturally specific services and information in various languages. Each health hub is hosted by a REACH partner organization and staffed with trained community health workers.
The services provided vary slightly according to the health hub location. These services include but aren't limited to
- blood pressure checks
- chronic disease self-management (CDSM)
- nutrition education
- group walks and more.

Health Hub Locations and Services
Each health hub holds walk-in office hours where community members can speak to a community health worker and receive certain services. Office hours may change, so please check this page regularly or contact the health hub you wish to visit for the most updated hours of operation.
