About REACH

REACH works with local organizations, other government programs and agencies, businesses and community members to prevent chronic disease and improve the collective health of Black and African communities (including immigrants and refugees) of Multnomah County.

Racial and Ethnic Approaches to Community Health (REACH) program works with the ACHIEVE (Action Communities for Health, Innovation and Environmental Change) Coalition, other government programs, local organizations and other community partners to improve the health of Black and African communities in Multnomah County. We focus on three areas: nutrition, physical activity, and breastfeeding. Our aim is to prevent chronic diseases (like diabetes and cardiovascular disease) through culturally-specific programming and community engagement.
