2025 Plan Changes
Premium Cost Changes:
- Moda Medical Plans: +10.0%
- Kaiser Medical Plans: +4.13%
- Kaiser Senior Advantage: +1.7%
- Delta Dental 50: +4.6%
- Kaiser Dental 15: +0.9%
- Willamette Dental: -3.0%
Moda Medicare Advantage Plan:
- Multnomah County will no longer have a Medicare Advantage option through Moda. If you are currently enrolled in the Moda Medicare Advantage plan, you will need to enroll in a different Medicare plan beginning January 1, 2025.
Moda Medical Plans:
- The Pharmacy Benefit Manager (PBM) is changing from WellDyne to Moda.
- NOTE: CVS pharmacies are now excluded on this plan.

- Insulin maximum cost share is decreased from $85 to $35 per 30 day supply and from $255 to $105 per 90 day supply
- Colonoscopy, including polyp removal, is covered for members age 45 and over as preventive care (100%) as recommended by their provider. Previously limited to every 10 years
Kaiser Medical Plans:
- Bariatric surgery services may be performed as an inpatient or outpatient procedure. Previously limited to inpatient
- Members without a designated primary care provider will automatically be assigned to a Kaiser Permanente primary care provider
Kaiser Senior Advantage (KSA):
- The Senior Advantage fitness vendor will transition from Silver & Fit to the One Pass fitness program
- Medicare will have a $2,000 prescription (Part D) maximum out-of-pocket. Multnomah County's KSA plan has very low co-pays for prescription drugs, so it is highly unlikely that any members will be impacted by this change
- Members without a designated primary care provider will be assigned to a Kaiser Permanente care provider
Moda Delta Dental Plan:
- Sedation Services are now covered under restorative benefits at 80% for members with intellectual or developmental disabilities
Kaiser Dental Plan:
- The custom lab-made night guard benefit is changing from a 10% coinsurance to a 35% coinsurance (member pays), limited to one custom lab-made night guard every five years. Kaiser offers a high-quality over the counter night guard option for members at $20 per night guard
Willamette Dental Plan:
- No changes
