On December 23, 2024, the Paperwork Burden Reduction Act was signed into law by the US President. As a result, employers and insurance providers are no longer required to mail 1095-C forms to covered individuals and full-time employees unless specifically requested.

The 1095-C form provided by Multnomah County advises you that Multnomah County medical coverage is considered to be qualified health coverage under the Affordable Care Act.

This information is already reported to the IRS on your behalf, which means that the 1095-C form is only for your records and is not something that you need to provide to the IRS when you file your federal income taxes.

However, we understand that some tax preparers are still requesting a copy of this form to complete your income tax preparation. 

If you need to obtain a copy of your form 1095-C, you can do so via the following options:

Please note that if you were enrolled in one of Multnomah County’s Kaiser medical plans, you may also want to obtain a copy of your 1095-B form in addition to your 1095-C form. You will need to contact Kaiser Permanente directly or log onto your Kaiser member portal to obtain a copy of your 1095-B form. Moda participants are only issued a 1095-C form.
