Order Ahead Replacement Ballot Request - November 5, 2024 General Election
Request a Replacement Ballot service will be available beginning on Oct. 24, 2024.
To complete the request form, please be prepared to share the following:
- Name (First & Last)
- Phone number
- Email address
- Residence address
- How you want to receive your ballot
Please note:
- The replacement ballot form is for Multnomah County registered voters. If your residence is in another county, please contact that county Elections office.
- If you plan to pick up a replacement ballot for another person, that person MUST complete a signed note explicitly giving you permission to do so. You MUST bring a copy of the signed note or form with you to pick up a ballot on their behalf. Please complete this optional form authorizing you to pick up the ballot, or bring in your own signed note with the same information.
