Bringing home a new baby is filled with change and big feelings. You don’t have to figure out those first few weeks alone!
Before you start
A Family Connects nurse can help welcome a new baby into your life. We'll come to your home around 3 weeks after your baby is born. We'll check in with you and baby, answer questions and see if there’s anything you need.
We have nurses who speak both Spanish and English. Interpreters available.
Currently available to parents who are patients at East County or Rockwood health centers.

Who We Serve
Any parent or caregiver with a newborn who goes to East County or Rockwood health centers.
It doesn’t have to be your first baby. It can also be an adoption or foster child.
There are no citizenship questions. Receiving services won’t affect your citizenship status.
No Cost
There are no income or insurance requirements. You won’t be billed.
What to do
1. Scheduling a visit
If you get care at East County or Rockwood Health centers, we will contact you to schedule a visit.
You can also fill out our referral form and we'll contact you.
2. How to prepare for your visit
No preparation needed! You might think about questions or a specific issue to ask the nurse. Otherwise, all you need to do is be there with your baby, plus anyone else in your family you would like at the visit.
You don't need to worry about cleaning your house or changing out of your comfy clothes.

No. We can meet at another location that works for you, or online.
