      • Request For Ballot Title SEL 805 - used by County and Special District to request ballot title and for Multnomah Co Elections to prepare and publish Public Notice. This form must be submitted no later than the 80th day before an election.
      • Notice of County Measure SEL 801 - used by County when public notice is complete or court has adjudicated title.
      • Notice of City Measure SEL 802 - used by City and sent to Multnomah Co Elections when City public notice is complete or court has adjudicated title.
      • Notice of District Measure SEL 803 - used by Special District when public notice is complete or court has adjudicated title.
      • Measure Explanatory Statement (225.97 KB) - used by district/jurisdiction when filing Explanatory Statement for County/City/District Measure.

Data Requests for Candidates & Campaigns

Please call Multnomah County Elections at 503-988-3720 if you have any questions. Data can be ordered from Multnomah County Elections by completing the appropriate form below with a $25 set-up fee (cash/check), and then the remainder paid when the disk is ready for pick-up.

Orders can also be placed with the Oregon Secretary of State's office at 503-986-1518. Orders placed through the Secretary of State's office can be done by phone and paid with a credit card. Fees are the same.
