Service Area Notice

Coverage is specific to Kaiser facilities in the Pacific Northwest (PNW). To be eligible for enrollment on Kaiser medical and dental plans, the policy holder must live or work within the Kaiser-defined PNW service area. Moving out of the PNW Kaiser service area may result in becoming ineligible for the plan and necessitate enrollment onto the Moda medical/dental or Willamette Dental plan/s. 

Kaiser may update their service area mid-year (See list of current service area zip codes below). If your primary address is identified as being out of the Kaiser service area, we will contact you to determine next steps. If you have questions about your eligibility, please contact us.

Center for Black Health & Wellness (Primary Care)

Kaiser's Center for Black Health and Wellness is located at their East Interstate Medical Office in North Portland. This clinical care center is focused on prevention, early detection, and chronic disease management. The Center’s goal is to close care gaps, improve measurable health outcomes, and support healthier futures for everyone they serve.
