Note: To be eligible for enrollment on Kaiser medical and dental plans, the policy holder must live or work within the Kaiser-defined service area at least 50% of the time. Moving out of the Kaiser service area may result in becoming ineligible for the plan and necessitate enrollment onto our Moda medical/dental or Willamette Dental Group plan/s. Kaiser may update their service area mid-year (See list of current service area zip codes below). If your primary address is identified as being out of the Kaiser service area, we will contact you to determine next steps. If you have questions about your eligibility, please contact us.
