Short Term Disability Insurance provides partial salary replacement in the event an employee is unable to work due to a non-work related accident or illness.

Eligibility:

You are eligible for coverage if you are an employee working 32 hours or 3 10-hour shifts per week and a member of AFSCME Local 88, JCSS, IBEW Local 48, ONA, and IUPAT Local 1094.

Cost and Weekly Benefit:

Premiums are paid by the employee via payroll deduction every paycheck and are based on both age and earnings.  Rates increase automatically when employee reaches new age and/or wage bracket.  Weekly benefit is based on employee's wage bracket.  Benefits on an approved claim begin after 30 days of sickness or injury, and may continue for a period of up to 9 weeks.

How to Apply

You may apply for coverage without answering any medical questions on the Evidence of Insurability form if you apply within 31 days of your hire date or applicable eligibility date (i.e. change of FTE into eligible status: PT to FT).
