The Department of County Human Services, Mental Health and Addiction Services Division (MHASD) is seeking one provider to facilitate continued reduction of the waitlist for civilly committed patients awaiting state hospital placement by providing care coordination services. The State of Oregon has stipulated that funds be used, immediately, for services that target adults with serious and persistent mental illness, and that they be used to: (1) facilitate persons moving from acute or long term care services to community based services; (2) reflect evidence-based case management practices; and (3) assist clients who are not already funded through the Mental Health Organization capitation.

1. Providing Services - Please demonstrate how your agency will provide the services identified in the
scope of services in a sustainable manner. What measures will you take to minimize impacts to the
environment in the delivery of services? Please provide specific examples, metrics, details of efforts,
practices, and/or processes. What sustainability innovations does your agency have to offer in the delivery
of services?
2.Culturally Competent Staffing – How will your agency ensure staff are culturally competent to allow
them to work with diverse client populations? How do you assess the cultural competency of your staff?
What organizational projects or initiatives have been developed to address the development of a culturally
competent staff and how have those been implemented?
3. Sustainable Business Practices - Please demonstrate your agency’s sustainable business
practices, which could include, but are not limited to a formal sustainability program and / or policies
covering ; energy conservation plans; water conservation policies; and a green cleaning policy. Please
provide specific examples, metrics, and details of practices and include copies of reports, policies or plans if
available.
Please specifically detail how you incorporate recycling and sustainability measures in the in daily
operations of your agency.
Please also specifically detail ways you will optimize travel to emergency departments. This could include
use of public transportation, using mobile devices to decrease number of trips, and/or mapping route for
fuel efficiency.
